Does Tirzepatide Make You Tired?

What the clinical trials show about tirzepatide and fatigue: how common it is, why it happens, when it hits after a dose, how long it lasts and what helps.

Tirzepatide is the once-weekly peptide behind Mounjaro and Zepbound. It activates two gut-hormone receptors, GIP and GLP-1, that turn down appetite and slow digestion, and for research it is supplied as a freeze-dried powder; Simms Research lists it as Tirzepatide (GLP-2 TZ). Energy is one of the first things people notice changing in the early weeks, because eating much less than usual shows up quickly in how the body feels.

People preparing for tirzepatide ask the same few questions. Does tirzepatide make you tired? Why does it happen, when does it hit after a dose, and how long does it last? This article answers each one with the clinical trial data, along with what helps, so it’s clear what’s normal from the first dose.

Does tirzepatide make you tired?

For most people, no. In the Zepbound weight-loss trials, SURMOUNT-1 and SURMOUNT-2, 2,519 adults took tirzepatide for up to 72 weeks. Fatigue, which the trials counted together with weakness, lethargy and malaise, was reported by 5–7% of them against 3% on placebo, so tirzepatide added only a few cases per 100 people.

The rate rose slightly with dose, from 5% at 5mg to 7% at 15mg. In the diabetes trials behind Mounjaro, fatigue didn’t reach the label’s list of common side effects at all.

Over the longer run, energy tends to improve. In SURMOUNT-5, people on tirzepatide improved on every domain of the SF-36 health survey at 72 weeks, including vitality, its energy and fatigue scale, and across both drugs in that trial the people who lost the most weight improved the most. In the sleep apnea trials, tirzepatide also improved how people rated their sleep and daytime functioning.

Why does tirzepatide make you tired?

Eating far less. This is the main cause. Tirzepatide can quiet appetite so much in the days after a dose that calories, carbohydrate and protein all drop well below what the body is used to, and energy follows intake.

Not drinking enough. Thirst drops along with appetite, and any nausea or diarrhea costs fluid on top of that. Mild dehydration feels like tiredness, a heavy head and dizziness, which 4–5% of people on tirzepatide reported against 2% on placebo.

Low blood sugar, mainly with other diabetes drugs. Tirzepatide’s insulin effect switches off as glucose falls, so on its own it rarely drops blood sugar too far: readings below 54 mg/dL occurred in 0.3% of people without diabetes. The risk sits with insulin or a sulfonylurea taken alongside it.

Lower blood pressure. As weight comes off, blood pressure often falls. Low blood pressure affected 1.6% of people on tirzepatide, more often those already taking blood pressure medication, and it can feel like fatigue or lightheadedness on standing.

When does tirzepatide fatigue happen?

Tirzepatide peaks in the blood about 24 hours after an injection (anywhere from 8 to 72 hours), which is why the tiredness people describe usually lands the day after the shot rather than the same day. It is most noticeable in the first weeks, before the body reaches a steady level after about four weekly doses, and again for a few weeks after each dose increase.

How long does tirzepatide fatigue last?

Within a week, the low-energy stretch usually lasts one to three days after the injection and lifts well before the next one. Over the course of treatment, it fades as the body adjusts to each dose, typically within the first several weeks on a dose. By week 72 of SURMOUNT-5, vitality scores were higher than at the start.

How to stop feeling tired on tirzepatide

Drink early and add electrolytes. Fluids with electrolytes on injection day and the day after cover the stretch when thirst is lowest.

Eat protein and complex carbohydrates, even in small amounts. Small, regular meals keep intake from crashing when appetite is lowest. A 2025 joint advisory from four obesity and nutrition societies notes proposed protein targets of 1.2–1.6 grams per kilogram of body weight a day during active weight loss.

Time the injection. Many people inject in the evening or before a lighter day so the low-energy day lands where it costs least.

Keep moving. Light activity and resistance training help energy and protect muscle while weight comes off.

Pace the dose increases. The approved schedule raises the dose by 2.5mg no sooner than every four weeks, and the label allows staying on a lower maintenance dose when a higher one isn’t tolerated.

Tirzepatide vs semaglutide fatigue

The labels aren’t a head-to-head comparison, but they give a useful reference. On Wegovy (semaglutide 2.4mg), fatigue was reported by 11% of people against 5% on placebo. On tirzepatide it was 5–7% against 3%. In SURMOUNT-5, the head-to-head trial, both drugs improved quality-of-life scores, and tirzepatide improved general health scores more.
