Tesamorelin Benefits and Results: What the Trials Proved (2026)

What the clinical trials of tesamorelin showed for belly fat, liver fat, cholesterol, muscle and the brain, how fast each result came, and what it does not do.

Most growth hormone peptides are sold on what they ought to do. Tesamorelin is the exception. Its benefits were measured in randomized trials, on CT scans, liver biopsies and memory tests, and the first of those trials won it approval in 2010, as Egrifta, to shrink the abdominal fat that gathers in some adults after years of HIV therapy.

That record cuts both ways. It proves a short list of results very well, and it also shows where the popular claims outrun the data. The scale barely moves on tesamorelin, about three in ten people in the approval trials lost less than 8% of their visceral fat, and a 2025 trial of thinking and memory in HIV found no edge over standard care. Simms Research sells tesamorelin as research powder, a 10mg or a 20mg vial, and its current batch tested between 99.49% and 99.63% pure in each of three vials weighed by an independent ISO-certified lab.

What is tesamorelin and what does it do?

It makes the pituitary release more of its own growth hormone. Tesamorelin is a lab-made copy of growth-hormone-releasing hormone (GHRH), the signal the hypothalamus sends to the pituitary, built from all 44 of its amino acids with a short fatty cap that stops enzymes from clipping it. The pituitary answers in its usual pulses, and the liver turns that growth hormone into IGF-1. In the first phase 3 trial, IGF-1 rose 81% on 2mg a day while it slipped 5% on placebo (Falutz, New England Journal of Medicine, 2007).

Working through the body’s own signal is what separates it from injected growth hormone. The feedback that caps each pulse stays in place, and two weeks at 2mg in 13 healthy men left insulin-stimulated glucose uptake where it started (Stanley, Journal of Clinical Endocrinology and Metabolism, 2011). How it differs from the shorter 29-amino-acid version of the same hormone is covered in Sermorelin vs Tesamorelin.

Tesamorelin for belly fat and visceral fat

This is the benefit tesamorelin was approved on, and it holds outside HIV. Pooled across both phase 3 trials, visceral fat, the layer packed around the organs, fell 24cm² in 26 weeks while it rose 2cm² on placebo, and patients and their doctors both rated the belly’s profile better. Those who kept dosing to week 52 held a 17.5% drop with a waist 3.4cm smaller (Falutz, Journal of Clinical Endocrinology and Metabolism, 2010). In 60 adults with abdominal obesity and no HIV, a year at 2mg shrank visceral fat while it grew on placebo (Makimura, Journal of Clinical Endocrinology and Metabolism, 2012).

The average hides a split. About seven in ten trial participants lost at least 8% of their visceral fat, the cut-off for a response set before the trials began (Lake, AIDS, 2021). Those responders got the metabolic gains too: triglycerides fell 0.6mmol/L against 0.1 in non-responders, fasting glucose held steadier and adiponectin, a marker of healthy fat tissue, improved (Stanley, Clinical Infectious Diseases, 2012). People with metabolic syndrome or triglycerides above 1.7mmol/L were the likeliest to respond (Mangili, PLoS One, 2015).

The fat that stays changes as well. On CT it grew denser in both the visceral and the under-skin layers, a sign of smaller, healthier fat cells, even after allowing for how much was lost (Lake, AIDS, 2021). The week-by-week course of the visceral fat drop is mapped in the tesamorelin dosage chart.

Tesamorelin for fatty liver

Yes. Two randomized trials cut liver fat, and the longer one slowed scarring. In 61 adults with HIV and fatty liver given 2mg or placebo for a year, paired biopsies showed fibrosis advancing in 2 of 19 on tesamorelin against 9 of 24 on placebo (Stanley, Lancet HIV, 2019). It held existing scarring in place rather than reversing it.

The biopsies showed why. Gene sets for oxidative phosphorylation, the way liver cells burn fuel, were turned up, while those for inflammation, tissue repair and cell division were turned down (Fourman, JCI Insight, 2020). Liver enzymes follow the same line: among phase 3 responders who started with ALT or AST above 30U/L, ALT fell 8.9U/L against a rise of 1.4 in non-responders, and the gain lasted to week 52 even in those switched to placebo (Fourman, AIDS, 2017). It also works for people on today’s integrase-inhibitor HIV drugs, which came after the approval trials: liver fat fell 4.2 points against 0.5 on placebo (Russo, AIDS, 2024). How that result sits next to retatrutide’s is set out in Tesamorelin vs Retatrutide.

Tesamorelin for cholesterol, triglycerides and the heart

Triglycerides move the most, cholesterol a little, and the artery-wall result comes from one trial. In the first phase 3 trial, triglycerides fell 50mg/dL while they rose 9 on placebo, and the ratio of total to HDL cholesterol improved by 0.31 while it worsened by 0.21 (Falutz, New England Journal of Medicine, 2007).

The heart markers come from the trial in adults with obesity and no HIV. After 12 months at 2mg the wall of the carotid artery, measured by ultrasound, had thinned by 0.03mm while it thickened on placebo, C-reactive protein fell, triglycerides dropped 26mg/dL against a 12mg/dL rise, and fasting glucose and HbA1c stayed flat (Makimura, Journal of Clinical Endocrinology and Metabolism, 2012). In people with type 2 diabetes, 12 weeks left insulin response and HbA1c unchanged while total and non-HDL cholesterol fell on 2mg (Clemmons, PLoS One, 2017). The liver trial added a quieter immune system: 13 circulating proteins tied to T-cell and monocyte activation went down and none went up (Stanley, Clinical Infectious Diseases, 2021). These are markers on the way to heart disease; no trial has yet run long enough to count heart events themselves.

Does tesamorelin help build muscle?

Some, and it builds better muscle more than bigger muscle. Across five randomized trials, lean mass rose 1.42kg more than on placebo, with no training program attached (Badran, Obesity Research and Clinical Practice, 2026). CT scans of the phase 3 responders show where it went: all four trunk muscle groups measured grew denser, meaning less fat threaded through them, and the rectus and psoas grew by just under half a square centimeter each in cross-section (Adrian, Journal of Frailty and Aging, 2019).

That is a lean-tissue result in middle-aged adults with HIV, not a bodybuilding one, and no trial has tested trained athletes. The first trial to pair tesamorelin with exercise, in 100 sedentary adults aged 50 to 80 with HIV over 24 weeks of home training, published its design in 2026 (Erlandson, BMJ Open, 2026). For a bigger growth hormone pulse around training, Tesamorelin vs Ipamorelin covers the ghrelin-side peptide most often added.

Tesamorelin benefits for the brain

Promising in older adults, unproven beyond them. In 152 adults aged 55 to 87, 66 of them with mild cognitive impairment, 1mg injected 30 minutes before bed for 20 weeks beat placebo on overall cognition, most clearly on executive function, the planning and task-switching skills, with a trend on verbal memory (Baker, Archives of Neurology, 2012). Healthy volunteers and those with impairment gained alike.

A brain-scan substudy of 30 of them found GABA, the main calming messenger, higher in all three regions measured, and myo-inositol, a marker linked to Alzheimer disease, lower in one (Friedman, JAMA Neurology, 2013). The follow-ups have been weaker. In 73 adults with HIV, six months at 2mg trimmed the waist 2.7cm more than standard care, but cognition improved no more than on standard care (Ellis, Journal of Infectious Diseases, 2025), and a 10-week pilot at 1mg in 22 adults found no significant change (Stewart, eNeurologicalSci, 2026).

What tesamorelin does not do

It doesn’t move the scale, thin the fat you can pinch, or keep working once it stops. The label calls it weight neutral and not a weight-loss drug (Egrifta WR label, FDA, 2026); across five trials BMI didn’t change (Badran, Obesity Research and Clinical Practice, 2026), and fat just beneath the abdominal skin differed from placebo by 0.6% (Falutz, Journal of Clinical Endocrinology and Metabolism, 2010). For weight itself, Tesamorelin vs Retatrutide sets it against a compound that moves the scale.

Sleep didn’t change in the 10-week pilot that tracked it, and neither did fatigue (Stewart, eNeurologicalSci, 2026). Skin, hair and testosterone aren’t among the outcomes its trials report, so those claims borrow from growth hormone in general. On appetite, it works on the GHRH receptor, not the ghrelin receptor behind the hunger pang of peptides such as ipamorelin. And the visceral fat comes back within six months of stopping.

Tesamorelin before and after: what changes, and how fast

IGF-1 climbs inside two weeks, the waist over three to six months, the liver over a year. After 26 weeks at 2mg, the before-and-after picture in the trials is a waist 2 to 3cm smaller and a flatter belly on the scan, a body weight within half a kilo of the start because lean tissue replaced the fat, and fasting triglycerides lower. The brain results took 20 weeks. The full timeline, scan by scan, is in the tesamorelin dosage chart.

Who tesamorelin suits

What the trials proved is narrow and solid: less fat around the organs and in the liver, with lower triglycerides and a little more lean tissue alongside, in the roughly seven in ten who respond. One trial each adds a thinner artery wall, lower CRP and sharper executive function in older adults. Weight loss, skin, hair and sleep aren’t on the list.

That makes it a fit for research on visceral and liver fat, on a thick middle over slim arms and legs, and on metabolic syndrome, where response was strongest. It is the wrong tool when the scale is the target, where Retatrutide (GLP-3) fits, or when a low-cost nightly growth hormone pulse is the goal, where Sermorelin at $78 or Ipamorelin at $74 fits. The fat and liver results above all come from 2mg a day, the cognition result from 1mg; the units for each are in the tesamorelin dosage chart.

At Simms Research, Tesamorelin costs $95 a 10mg vial and $158 a 20mg vial. The current batch went to an independent ISO-certified lab as three separate vials, which came in at 99.63%, 99.52% and 99.49% pure and held 10.84mg to 11.36mg each, with identity confirmed by LC-MS and clean endotoxin, heavy-metal and sterility results. The full certificate, beside the older May batch, sits on the tesamorelin lab results page.

Frequently asked questions

Does tesamorelin cause cancer?

No trial has shown that it does. Its approval trials enrolled no one with a cancer history, and the label rules it out during active cancer because the growth hormone it releases is a growth factor (Egrifta WR label, FDA, 2026).

Is tesamorelin the same as HGH?

No. HGH is the hormone itself; tesamorelin asks the pituitary to release its own, in pulses, with the body’s feedback still in charge.

Does tesamorelin work for women?

The same way. Most phase 3 volunteers were men, but the liver, obesity and cognition trials enrolled both sexes and reported one result for all.
