Sermorelin vs Tesamorelin: Belly Fat, Muscle and Cost

Sermorelin vs tesamorelin compared: what each one is, the belly-fat and muscle evidence, results in women, side effects, units and cost per month, and which fits which goal.

Sermorelin and tesamorelin carry the same message to the pituitary: release growth hormone now. Both are copies of GHRH, the hormone the brain uses to send that signal, and both reach the same receptor. One is the short original from the 1980s, sold for years as a children’s growth drug. The other is the full-length hormone with a protective cap, built two decades later and approved for a single job, shrinking deep belly fat.

Simms Research sells both: Sermorelin as a 10mg vial for $78, Tesamorelin as 10mg for $95 or 20mg for $158. Which one fits comes down to the result the research is after and what each costs to run for a month.

What is sermorelin, and what is tesamorelin?

Natural GHRH is 44 amino acids long, but its first 29 carry the full growth-hormone signal. Sermorelin is exactly those 29. As Geref it was approved in 1990 as a test of how much growth hormone the pituitary can release, and in 1997 as a nightly treatment for children short on growth hormone. EMD Serono discontinued it in 2008, and a 2013 FDA notice found it had not been pulled for safety or effectiveness.

Tesamorelin keeps all 44 amino acids and adds a small six-carbon chain, a trans-3-hexenoyl group, to the first one. Theratechnologies took it to approval as Egrifta in 2010, to reduce the belly fat that builds up in adults with HIV-associated lipodystrophy, and it is still sold for that today as Egrifta SV and Egrifta WR. No other GHRH-type drug holds a current US approval.

What is the difference between sermorelin and tesamorelin?

Survival, and the trial record that came with it. In the blood, an enzyme called DPP-4 snips the first two amino acids off GHRH within minutes, and the clipped hormone no longer works. Sermorelin is cut at the same spot. Tesamorelin’s cap sits on exactly that end and blocks the cut, so more of each dose stays intact on its way to the pituitary. At the receptor itself they are evenly matched: Egrifta’s label rates tesamorelin as about as potent as the body’s own GHRH.

That shows up in the doses. Sermorelin runs in micrograms, a few hundred at bedtime; tesamorelin runs in milligrams, 1 to 2mg a day. Neither lingers, since both clear the blood within minutes, and neither overrides the body’s brakes: growth hormone still comes out in pulses, and rising IGF-1 still damps the next one.

The larger gap is evidence: tesamorelin’s results come from trials of hundreds of people, sermorelin’s adult results from studies of a few dozen.

Sermorelin vs tesamorelin for belly fat

Tesamorelin, by a wide margin. Its trials were built around belly fat, and they were large. Across two phase 3 trials, the first of them in the New England Journal of Medicine in 2007, 816 adults with HIV took 2mg a day or placebo for 26 weeks. CT scans of visceral fat, which sits behind the abdominal wall and wraps the organs, showed drops of 18% and 14%, while the placebo groups went +2% and −2%. Fat under the skin held steady, and the scale barely changed, since lean tissue grew as the deep fat shrank.

The result carries outside HIV. In a 12-month trial of 60 adults carrying excess belly fat whose growth hormone ran low, the 2mg group lost 16cm² of visceral fat while the placebo group added 19cm², and triglycerides fell with it. It also lasts only as long as the dosing: in the phase 3 extension, people moved to placebo at week 26 regained most of what they had lost within another six months.

Does sermorelin really burn belly fat?

A little, on the one trial that measured it. In a 2004 Harvard trial, 31 men with HIV lipodystrophy injected 1mg of Geref twice a day for 12 weeks. Trunk fat fell 0.4kg against a 0.2kg gain on placebo, and visceral fat shrank 19cm² against a 2cm² rise, a drop that fell just short of statistical significance. Steven Grinspoon’s group at Massachusetts General Hospital ran that trial, then went on to co-author tesamorelin’s phase 3 work.

The older-adult studies, at single nightly draws, found no change in body fat at all. Sermorelin’s belly-fat case rests on 2mg a day, nearly seven times the usual 300mcg research draw, and it is a modest one.

Sermorelin vs tesamorelin for muscle growth

Both add lean mass in the range of a kilogram, and tesamorelin has the firmer numbers. In its phase 3 trials, 26 weeks at 2mg added 1.2 to 1.3kg of lean mass while trunk fat fell. In a follow-up study from the 12-month obesity trial, the rise in IGF-1 tracked with faster muscle energy recovery after exercise.

Sermorelin’s best lean-mass result comes from the same 2004 trial: 0.9kg gained in 12 weeks at 1mg twice a day, against a 0.3kg loss on placebo. In older men, six weeks of 2mg a night improved two of six strength tests without changing muscle mass, and 16 weeks of a close variant raised lean mass in men but not women. For muscle, tesamorelin’s result comes from hundreds of people; sermorelin’s from a few dozen, at doses well above its usual nightly draw.

Sermorelin or tesamorelin for women

Either, at the same dose as men; neither label nor the usual protocols adjust the draw for sex. What differs is how much of the evidence includes women. They made up 14% and 16% of tesamorelin’s phase 3 trials and most of a 20-week trial of 152 adults aged 55 to 87, where 1mg at bedtime raised IGF-1 by 117% and cut body fat by 7.4%.

Sermorelin’s main data on women is a 1997 study of 10 women and 9 men aged 55 to 71, on a close variant of the 29-amino-acid chain each night for 16 weeks. Growth hormone, IGF-1 and skin thickness rose in both sexes. Lean mass, insulin sensitivity, well-being and libido improved in the men alone, and the authors concluded that the anabolic effects favored men. For a woman whose goal is body composition, tesamorelin has more female data behind it.

Sermorelin vs tesamorelin side effects

For most, the injection spot is where it starts and ends, on either peptide. Geref’s trials put soreness, swelling or redness where it went in at roughly one in six patients, and nothing else at even 1 in 100.

Tesamorelin’s trials were large enough to give rates against placebo. Over 26 weeks, the Egrifta WR label lists injection-site reactions in 17% against 6%, joint pain in 13% against 11%, and muscle aches and swollen hands or feet in 6% each against 2%. Those come with the growth hormone rise and settle or stop with the dosing. Blood sugar moved in a few: 5% reached a diabetic-range HbA1c by week 26, against 1% on placebo.

Sermorelin vs tesamorelin dosage and cost

Both take 2mL of bac water. Mixed that way, every 10 units on a U-100 syringe holds 500mcg from either 10mg vial, or 1mg from the 20mg tesamorelin vial. Sermorelin’s 200 to 300mcg at bedtime is 4 to 6 units. Tesamorelin’s 1 to 2mg a day is 20 to 40 units from the 10mg vial, or 10 to 20 from the 20mg. Cost follows the milligrams:

ProtocolUnits per drawOne vial lastsCost per 30 days
Sermorelin 200mcg nightly4 units28 days$84
Sermorelin 300mcg nightly6 units28 days$84
Tesamorelin 1mg a day, 10mg vial20 units10 days$285
Tesamorelin 1mg a day, 20mg vial10 units20 days$237
Tesamorelin 2mg a day, 10mg vial40 units5 days$570
Tesamorelin 2mg a day, 20mg vial20 units10 days$474

A sermorelin vial holds 33 to 50 bedtime draws, but a mixed vial keeps 28 days, so the calendar ends it first: one vial a month either way. At 2mg, the dose behind the visceral-fat results above, a month of tesamorelin runs five to seven times a month of sermorelin, and the 20mg vial takes about a sixth off it. 1mg costs half as much; in the one trial that compared the two doses, it raised IGF-1 but did not cut visceral fat beyond placebo. A month at 2mg also means three to six fresh mixes; at 2mL each, one $20 bottle of Research Water (10mL) handles five.

The sermorelin dosage guide and the tesamorelin dosage guide carry the full unit charts, timing, and how long a mixed vial keeps. For other mixes the peptide calculator does the math, and the peptide refrigeration guide covers the powder before and after mixing.

Ipamorelin vs sermorelin vs tesamorelin

Ipamorelin is the odd one of the three, and it explains why sermorelin and tesamorelin are not taken together. Both copy GHRH and work one receptor, so a second GHRH adds no new pathway, and no trial has paired them. Ipamorelin copies ghrelin, the hunger hormone made in the stomach, and releases growth hormone through the ghrelin receptor. That is the pairing that makes sense. Tesamorelin vs ipamorelin covers stacking it with tesamorelin, sermorelin vs ipamorelin the classic nightly pair, and CJC-1295 vs ipamorelin the ready-made blend.

Is tesamorelin better than sermorelin?

For fat around the abdomen, yes. For a nightly growth hormone pulse for a fraction of the price, sermorelin is the better buy.

Tesamorelin fits research aimed at visceral fat, liver fat or body composition. Alone among the two, it has large controlled trials showing deep belly fat coming off, in HIV and in abdominal obesity, with about a kilogram of lean mass gained alongside. The trade is price, $474 to $570 a month at the trial dose, and a result that holds only while the dosing continues.

Sermorelin fits research aimed at the growth hormone axis itself: a bedtime pulse added to the body’s own, IGF-1 up within two weeks, and a protocol that can run for months at well under $100 a month. Its record is older and smaller, and at the usual draw it leaves body fat where it is.

One GHRH, not two. Pick the one that fits the goal, and add a ghrelin-type peptide only if the research calls for a second pathway.

How the latest sermorelin and tesamorelin batches tested

Every vial tested held more than its label, and both batches averaged over 99.5% pure. Three vials from the current batch of each went to an independent ISO-certified lab on July 16, 2026: sermorelin’s averaged 99.64% pure with 10.64mg of peptide, tesamorelin’s 99.55% with 11.11mg. LC-MS matched both to the right molecule, endotoxins passed, no heavy metals turned up and sterility cultures stayed clear. So a tesamorelin draw holds about 11% more than the unit math above says, and a sermorelin draw about 6% more.

Full certificates, May 2026’s single-vial batches included, are on the sermorelin lab results and tesamorelin lab results pages, with every other compound in the lab results library; the guide to reading a peptide COA explains each figure.

Frequently asked questions

Which peptide is better than tesamorelin?

None, where visceral fat is the goal: no other growth hormone peptide has stronger evidence. Pairs such as CJC-1295 with ipamorelin release more growth hormone by working two receptors at once, but none has a belly-fat trial of its own.

What is better than sermorelin?

It depends on the goal. For belly fat, tesamorelin. For a longer signal from the same short chain, CJC-1295 without DAC, which is sermorelin with four amino acids swapped and a half-life of about 30 minutes instead of 11.

What is the strongest HGH peptide?

By results measured in people, tesamorelin: no other growth hormone peptide is approved as a treatment today on phase 3 outcome trials. By the size of a single growth hormone release, a GHRH-type peptide paired with a ghrelin-type one.

How long do sermorelin and tesamorelin take to work?

IGF-1 rises within two weeks on either. Tesamorelin’s visceral fat drop shows by 12 weeks and is clearest at 26. Sermorelin’s adult studies measured body changes at 12 to 16 weeks.
