Retatrutide Early Access: Who Qualifies and How the Program Works
Who qualifies for Lilly's retatrutide early access program, how a doctor applies, the one US trial still enrolling, and what each route to retatrutide costs.
When Eli Lilly said on August 3, 2026 that some patients could get retatrutide before FDA approval, it read like an open door to the most anticipated weight-loss drug in development. Retatrutide had just produced 28.3% average weight loss at 80 weeks in Lilly’s TRIUMPH-1 trial, and plenty of people already on Zepbound wanted to know how to move up.
The door is a single-patient expanded-access program, and it was written for very few of them. It takes a BMI of 35 or more after the top dose of an approved weight-loss drug, two serious obesity-related conditions already under treatment, and a doctor willing to file the request, since Lilly takes none from patients. Lilly hasn’t said what it charges.
Simms Research carries Retatrutide (GLP-3) as research powder in 10mg to 60mg vials, and its latest batch was tested in July 2026 by an independent ISO-certified lab.
Who qualifies for retatrutide early access
Adults whose obesity hasn’t given way to the strongest approved treatment and is already making them seriously ill. Lilly’s record lists five requirements and one exclusion, and a patient has to meet every one of them:
| Requirement | What the record says | What it means in practice |
|---|---|---|
| Age | 18 or older | No access for teenagers |
| Weight | BMI of 35 or more despite taking and tolerating the highest available dose of a weight-loss drug | Still at a BMI of 35 or more after reaching the top dose of a drug such as Zepbound |
| Complications | Two or more serious or life-threatening obesity-related conditions, already being treated | The first known case cited sleep apnea and high blood pressure in the lungs |
| Trials | Can’t join a retatrutide trial or a comparable one, by its criteria or for lack of a site in reach | A trial within reach that the patient fits comes first |
| Options | Every standard treatment, bariatric surgery included, talked through with the doctor | Surgery has to have been discussed, not done |
| Exclusion | A medical reason not to take retatrutide | Judged case by case, since retatrutide has no label yet |
Two words in the weight requirement do most of the sorting. “Highest available dose” means the patient climbed the whole ladder of a current drug, not a middle step. “Tolerance” means they stayed on it, so someone who stopped Zepbound over nausea doesn’t fit the wording as written, even with a BMI well over 35.
Who the criteria leave out
Most of the people asking. A patient doing well on tirzepatide who wants a bigger result, anyone with a BMI in the low 30s, and anyone who hasn’t reached the top dose of an approved drug are all outside it. Meeting every line still isn’t a yes: Lilly decides each case on its own, and its record says supply and geography play a part. The retatrutide vs tirzepatide comparison sets out what separates the two drugs for those who stay on tirzepatide.

How the retatrutide early access program works
The doctor asks, Lilly decides, and then the FDA and an ethics board sign off before any drug ships.
It starts with the treating doctor calling the Lilly Answers Center at 1-800-545-5979 or writing to Lilly’s clinical trials inbox. The program is listed on ClinicalTrials.gov as NCT07629401, “Pre-approval Expanded Access of Retatrutide.” Lilly reviews the patient’s medical file, and its expanded-access policy promises an answer within five business days once it has everything it asked for.
A yes from Lilly is only the first approval. The doctor then files a single-patient request with the FDA and has the treatment plan approved by an institutional review board, the ethics committee that oversees research on people. Lilly ships to the doctor, not the patient, and the doctor runs the treatment and the follow-up visits.
In practice, doctors who applied over the summer described weeks without a real answer, and one patient’s request reached a preliminary qualification only after reporters asked Lilly about it. Lilly hasn’t said how many patients it will take or how long each one can stay on the drug.
Is there a retatrutide early access sign up
No. Lilly runs no waitlist, patient form or sign-up page, and its record tells patients and families to go to their own doctor rather than to Lilly. Any page offering a place in line belongs to someone else.
Retatrutide expanded access and compassionate use
Compassionate use is another name for expanded access, and retatrutide’s first known case set its terms. In April 2026 Lilly supplied the drug to one 79-year-old patient, on a request from a senior clinician at the National Institutes of Health. Lilly posted the program’s record on June 5. Once STAT reported the single case on June 23, doctors began filing for their own patients, including patients who hadn’t lost enough on Zepbound.
On August 3 Lilly confirmed it was reviewing those requests, saying it was medically appropriate to supply retatrutide before approval to a limited number of patients who meet specific criteria and can’t enroll in a trial. The record was updated the same day, and on October 30 it still reads “Available.” Lilly’s October 29 quarterly results said nothing new about it.

How to get retatrutide through a clinical trial
Through one large trial in fatty liver disease. Lilly’s retatrutide weight-loss trials, TRIUMPH included, have all stopped enrolling, and on October 30 the only Lilly retatrutide trial recruiting in the US is SYNERGY-OUTCOMES (NCT07165028), open at 137 sites across 34 states.
It is built for adults with liver fat and early scarring, measured without a biopsy: liver fat of 8% or more on a scan, a liver-stiffness reading of 10 to under 20 kPa on a FibroScan, an ELF blood score of 9 to 10.8, and a BMI of at least 25. Losing more than 11 pounds in the three months before screening, an A1c above 10% or type 1 diabetes rules a person out.
The trade is time and chance. About 4,500 adults are assigned at random to retatrutide, tirzepatide or placebo for roughly 224 weeks, with 25 to 30 clinic visits, and participants aren’t told which they’re on. Afterwards an optional two-year extension puts everyone on retatrutide or tirzepatide, placebo group included. The trial’s ClinicalTrials.gov record lists every site, and a patient who fits one within reach doesn’t meet the early-access criteria.
Can you get a retatrutide prescription
Not yet. Retatrutide isn’t approved for any use, so no doctor can write a prescription that a pharmacy can fill, and that includes compounding pharmacies. The HHS policy signed on October 28, covered in the RFK peptides list, lets pharmacies compound six peptides on a prescription, and retatrutide isn’t one of them. The prescription route opens with FDA approval.
What retatrutide costs on each route
Lilly hasn’t priced it anywhere. It hasn’t said what it charges for expanded-access supply, and a list price only comes with approval.
Lilly’s self-pay Zepbound is the closest guide to the prescription price. On LillyDirect it costs $299 a month at 2.5mg, $399 at 5mg and $449 from 7.5mg to 15mg, as long as each refill comes within 45 days. Here is every route side by side:
| Route | Who it’s open to | What it costs | How long it takes |
|---|---|---|---|
| Expanded access | BMI of 35 or more after the top dose of an approved drug, two serious conditions, no trial in reach | Not published by Lilly | Lilly answers within five business days of a full file; FDA and ethics board approval follow |
| SYNERGY-OUTCOMES trial | Adults with higher-risk fatty liver disease and a BMI of 25 or more | Study drug supplied by the trial | Screening at a site, then about 224 weeks, possibly on placebo |
| Prescription | Patients a doctor prescribes it for, after approval | Not priced; self-pay Zepbound runs $299 to $449 a month | Lilly plans to file in early 2027 |
| Retatrutide (GLP-3) research powder | Laboratory research | $84 for 10mg to $259 for 60mg | Ships the same day on orders before 5PM Eastern |
When will retatrutide be available to the public
Not before 2027. Lilly plans to submit retatrutide to the FDA in the first quarter of 2027, and a standard review takes about a year from submission, so pharmacy supply comes after that. Early access ends at the same point: Lilly’s policy phases out its expanded-access programs once a drug is approved. The filing, the review clock and the likely launch window are covered in when retatrutide will be FDA approved.

How to get retatrutide now for research
For laboratory research it’s available today as freeze-dried powder. Simms Research sells Retatrutide (GLP-3) in 10mg ($84), 20mg ($129), 30mg ($159) and 60mg ($259) vials. The latest batch, tested on July 16, 2026 by an independent ISO-certified lab across three vials, came back 99.611% pure with 10.68mg in a 10mg vial, identity confirmed by mass spectrometry, and passed its endotoxin, heavy metal and microbial screens; all six published batches are in the retatrutide lab results. At the trials’ 4mg a week, one 20mg vial covers four weeks, and at 12mg a week one 60mg vial does. Orders placed before 5PM Eastern ship the same day, FedEx 2-Day is free over $200, and the Free Delivery Guarantee covers every order.
The retatrutide dosage chart has the trial schedules step by step, the retatrutide calculator turns any vial and dose into syringe units, and What Is GLP-3 (Retatrutide)? explains how the three hormone targets work. The side effects of retatrutide for women guide gives trial rates next to placebo.
Frequently asked questions
Who qualifies for the retatrutide early access program?
Adults with a BMI of 35 or more after taking and tolerating the highest dose of an approved weight-loss drug, with two or more serious obesity-related conditions under treatment, who can’t join a retatrutide trial and have discussed every standard option, surgery included, with their doctor.
Can I apply for retatrutide early access myself?
No. The treating doctor makes the request by calling Lilly at 1-800-545-5979, and Lilly sends patients who contact it directly back to their own doctor.
How long does Lilly take to answer?
Its policy is five business days after it has the full medical file. Doctors who applied in the summer of 2026 reported waits of several weeks, and the FDA and an ethics board still have to approve after Lilly does.
Does retatrutide early access cost anything?
Lilly hasn’t published a price for expanded-access supply or said how long a patient stays on it.
Can I switch from Zepbound to retatrutide early?
Only through early access, and only if the top dose of Zepbound hasn’t brought the BMI under 35 and the other requirements fit. Everyone else switches after approval, with a prescription.
Is retatrutide FDA approved?
No. Lilly plans to file in the first quarter of 2027, after TRIUMPH trial results through 2026 that reached 28.3% average weight loss at 80 weeks on the top 12mg dose.
Can a compounding pharmacy make retatrutide?
No. It’s an unapproved drug, and it isn’t on the list of six peptides HHS opened to pharmacy compounding in October 2026.
Is Simms Research legit?
